60-second summary

What this means for providers

Support at Home commenced on 1 November 2025, replacing the Home Care Packages Program and Short-Term Restorative Care Programme. It is a home-based service model with eight ongoing funding classifications, quarterly budgets and separate short-term supports. The Commonwealth Home Support Programme did not transition at that date.

Management questions and example measures are Centremetrics operational recommendations, not additional regulatory requirements.

Assessment determines a participant’s classification and approved services. Ongoing funding is allocated in quarterly budgets. Track the classification, available funding and planned services for each participant rather than relying only on an average across the organisation.

Current funding amounts and contribution rules should be checked in Department guidance. Unspent ongoing funds can carry over up to the greater of $1,000 or 10% of the quarterly budget; transitional Home Care Packages balances need separate visibility.
